The demolition of existing Side extension and the construction of a new two storey, Pitched-roof side extension with a new pitched roof porch. The construction of a new rear dormer and the construction of a 40m2 Rear Extension. Minor internal alterations to the existing dwelling.

Single storey extension comprising of a link corridor, storeroom and two GP consultation rooms to an existing GP surgery, alterations to the existing surgery front elevation, including new entrance door to existing porch, replacing current entrance door with a window. Works include internal layout changes to existing waiting area with proposed new reception and disabled access toilet.

New pitched roof over the converted garage and rear utility room and canopy over the front porch.
